Home from Hospital - Supporting Community Recovery
This service can start as soon as you’re ready for discharge from hospital. We’ll help you through the discharge process and then take you home. Once you are home, we can help with
- Light household tasks
- Basic food preparation
- Medication collection (from hospital or your local chemist)
- Shopping for essential items
This is a comprehensive service designed to make sure you’re getting everything you need to enjoy being back in your home. It starts with a home visit when we’ll discuss exactly what kind of help you need.
- Benefit and entitlement advice and check – find out which beneftis you can receive including Attendance Allowance, Carer’s Allowance, Pension Credit, etc.
- Safe & Connected Alarms – do you need a pendant alarm that can alert people if you fall? We can refer you.
- Dial-a-Ride & Taxicard – We’ll help you with your application
- Adult Social Care – If you need support from Adult Social Care, we can help you make contact
- Occupational Therapy – we can help arrange an assessment to help you determine what equipment you need to improve accessibility in your home.
- Support & Social Groups – Many medical conditions, like COPD and Memory, have specific support groups. Meeting other people who are facing the same or similar challenges can help you feel less isolated.
Befriending Service
On of our specialist befrienders will visit you for about an hour every week. They’re there to chat and give you some social contact when friends and family are distant. That way you won’t get too lonely, and can look forward to a visit.

The Home From Hospital service is funded by Enfield Council for 18+ Enfield residents who can benefit from additional support for up to 6 weeks.

Memory Lane Café and Carers Club. For anyone experiencing memory problems and their carers. Friday’s 10:45am-1:45pm. Takes place at 373 Bowes Road, Arnos Grove, London, N11 1AA.
